For a man, nothing can be more embarrassing than having breasts. But this can happen and does happen to a lot of men. Medically termed “gynecomastia” a term that originates from the Greek words for "women-like breasts" this is a combination of breast tissue and fat accumulation. The social stigma attached to males having excess chest skin or swollen breast tissue can be physically uncomfortable and psychologically. Boys and men face a lot of ridicule for the appearance of their chest which start looking like a female's chest causing them to avoid circumstances where the chest has to be bared.

What are the causes of gynecomastia?

    Gynecomastia is a common condition and affects an estimated 40 to 60 percent of the male population and its most common cause is pubertal. This condition is typically associated with heredity rather than various lifestyle choices, so even opting for a strenuous exercise regimen and adhering to a strict diet may not fix the problem. Certain pills such as anabolic steroids, medicines containing estrogen, alcohol, marijuana, etc., and other medical conditions including cancer, weakened liver function, to name a few, may prompt or add to enlarged male breasts.

Who are the right candidates for surgery?

  • Boys who are more than 18 years of age and have enlarged breasts.
  • Boys below the age of 18 having enlarged breasts and have psychological issues due to peer pressure.
  • Older men who have seen an increase in the size of their breasts due to increased weight.
  • Males having asymmetric breasts.
  • Men who have undergone breast reduction surgery but still have residual breast tissue.
  • Men who have enlarged breast secondary to steroid treatment taken during muscle building exercises.

What are the anesthesia requirements for gynecomastia surgery?

    The procedure is usually carried under local anesthesia with sedation or general anesthesia as a daycare procedure if all your baseline investigations are within normal limits. The baseline study should include an ultrasound of the breast to rule out any pathology.

How is the procedure carried out?

    The surgery is carried out through two keyhole incisions on the sides of your chest with the help of power-assisted liposuction technique. The excess fat and breast tissue are sucked out and after the surgery, you are made to wear a pressure garment for a period of four weeks. Sometimes there may be residual breast tissue which can be addressed through a periareolar incision and takes an average of two hours. Excess breast envelope or drooping NAC (Nipple-areola complex) can be addressed at the same time or at a later stage. This is usually required in Grade 3 gynaecomastia usually seen after massive weight loss.

    Results are lasting, although consequent obesity can produce a gynecomastia-like effect. Some of the benefits of surgery include a firmer, flatter, more contoured chest which may give the male patient a boost in self-confidence. There is short downtime, and you may resume working within 1 week unless you are committed to strenuous activities. There will be an incision around the nipple of the breast (areola) if the cut has been made to remove excess breast tissue but will fade over a period of time and be less visible. Bruising, swelling and burning sensation will be there after the procedure. To help with the healing process, the patient will wear a stretchy pressure garment for 3 to 6 weeks and direct exposure to the sun must be avoided.

    Other considerations include numbness for a brief amount of time or lack of feeling that might last up to a year. Uncommon complications like infection, skin damage, fluid accumulation, bleeding, scarring or pigment changes might show up. Post-operative asymmetry, while rare, is likely to happen; the following procedure may be needed to eliminate additional tissue.

What are the post-procedure precautions?

    You will be discharged in the evening of surgery. From next day onwards you are permitted to do restricted physical activity. By the end of two weeks, you will be able to do most of your activities. The pressure garment is to be worn for a period of 2 weeks for 24 hours and thereafter only at night for another two weeks. This is important as it helps the skin to drape over your body without leaving any dead space.
