Fat is a sign of youth. The importance of fat grafting is opening up avenues in giving patients a more youthful look after undergoing various cosmetic surgeries. Most of the patients who undergo facelift surgery have tighter skin. The results may not give them the youthful look they want and this happens due to fat atrophy on the facial regions. In such cases, adding fat to the deficient areas not only adds volume but also restores the youthful look.

Fat Grafting

    It is a process of transferring fat from one area to the area which is devoid of volume. The survival of fat depends on the vascularity it develops over time. The most common area from where the fat is harvested in the lower abdomen area. The lower abdomen area consists of fat which is resistant to resorption overtime and it contains a lot of stem cells which tends to enhance the property of fat when it is grafted.

    Harvesting of correct sized fat particles becomes vital. It is a general understanding that smaller the fat cells which are harvested, the better is the uptake. Larger fat cells have fewer chances of survival once they are grafted. The fat cells develop blood supply around their peripheries and survive on it.

Protocol of Fat Grafting

    Harvesting and transfer of harvested fat must be performed carefully.

    For large volume fat harvest larger liposuction cannulas are used and for smaller harvests, finer cannula system is used. The lipoaspirate must be cleansed of unwanted tissues.

    In the malar and temporal regions ‘dry’ graft (non-diluted) is the preferred choice, but in the periorbital area a more liquid (diluted) form of the graft is utilized). For a liquid graft, a ration of saline: fat is usually 70:30. Using a liquid graft in the periorbital region has less likelihood of lumps developing under the thin skin of the lids.

    Injection of the fat into the recipient site can be undertaken by several means. The cannula is the safest way to inject the fat into the recipient site. Using cannula also reduces the chances of fat getting into blood vessels which would lead to severe complications. One of the safe ways to inject the fat using any mechanism is to inject it while withdrawing the syringe. This further reduces the chances of fat entering into intravascular spaces.

Contraindications

  • Patients who do not have enough fat for grafting.
  • Patients who underwent dermal fillers recently.

Complications

    The most common complication is resorption of fat. It might need few touches up procedures. Fat getting infected is another complication. It has to be managed with appropriate medications and surgical intervention if needed. One of the most important intraoperative complications is the fat entering into the intravascular space, which would be a very serious incident.

After the procedure

  • Minimizing the pressure to grafted areas.
  • High carbohydrate intake for 4 weeks is important as it helps with graft uptake.
  • Swelling is normal.
  • Volume changes are visible after 4 months.
  • Appreciation of results may take up to 6 months.

    It offers an excellent and a more permanent alternative to dermal fillers. Results last longer once the fat cells develop blood supply around them. In this regard, fat harvest plays an important role. Maintenance of the youthfulness after fat grafting may need few maintenance sessions later.

Fat grafting, if carried out in the right tissue plane is very safe in the face. It is important to stay away from unsafe areas on the nose and the angle of mouth so that the fat cells do not travel into the blood vessels causing blockage and tissue necrosis.

As answered before, if live fat cells are transferred in perfect circumstances onto a well-vascularized tissue in the face, they will survive, develop a blood supply and will be there permanently.
