There are different sizes and shapes of breasts as well as implants hence it is very difficult for an inexperienced physician to pick the right size for you. Smooth implants are associated with a slightly increased risk of capsular contracture whereas the textured implants have been recently known to increase the risk of ALCL in breast implant patients.

Breast Implants are usually placed through 3 access incisions, namely, Inframammary (below the breast and the most commonly used), Periareolar (less common usage due to the destruction of 50% of milk ducts) and Axillary (very uncommon in India due to the tendency of scarring and pigmentation). There is another technique which is called TUBA and involves placing a saline implant through an umbilical incision. This technique is rarely practiced today. The safest technique and the most commonly used is the Inframammary technique because the scar is well hidden in the crease, bleeding control and placement of the implant is easier, it is easy to replace the implant and the incision is away from the nerves and vessels supplying the nipple.

Once you have met Dr. Mohan Thomas and discussed your requirement, he will assess your chest and advise a suitable implant size for you. Suitable size pockets are created and breast sizers are placed to assess the adequacy. The implant is then placed in the pocket by a single touch technique pioneered by Dr. Mohan Thomas to reduce the risk of infection. Closure and dressing are then done and the patient is sent home in a few hours.

The only alternative to Breast Implant for augmentation is to use your own fat injected in small aliquots so that the injected fat develops a blood supply. Fat is harvested in a sterile manner from your abdomen and thighs. Live fat cells and stem cells are then separated and loaded onto small syringes. These are then injected systematically in different quadrants of the breast. Dr. Mohan Thomas only injects enough fat to increase the breast size by one cup size only at a time. A further increase can be undertaken 3 months later.

The greatest concern about this technique is the necrosis, destruction, and scarring of the grafted fat creating a lumpy breast. Moreso is very difficult to differentiate between a scarred area and early malignancy on Mammogram.

Breast Implant surgery poses various risks which include

1. Scar tissue that distorts the shape of the breast implant known as capsular contracture
2. Breast pain
3. Infection
4. Changes in nipple sensation
5. Implant leakage or rupture

Before considering breast surgery you would need the following:

1. Baseline mammogram or a sonomammogram is needed before the surgery.
2. Aspirin and other blood thinners have to be stopped prior to the surgery.
3. If you smoke, you need to stop smoking 2 weeks before the surgery.

Soreness and swelling remain for a few weeks after the surgery. Bruising is also possible. Scars fade away over time but don't disappear completely. You might be able to return to work within a few weeks. Avoid strenuous exercises and any activity that could raise your pulse or blood pressure for at least two weeks.
